Description

Hourly and minimal on call (4 times a year).

Residential Program Director  Program includes one a 24-hour, four female residence with one vacancy. Requiring medical assistance, appointments, and community support needs. Generally double staffed including overnights. Nursing Dept. assists with medical documentation. In addition, one independent living women, in Plymouth, requiring employment & community supports approx. 15 hours a week. Seeking a strong leader with good organization, MAP fluency and delegation skills.

Hours: 40 hours per week Monday Through Friday 9:00am to 5:00pm

Job Responsibilities

 Responsibilities:

  1. Directors are responsible for the day‑to‑day functioning of the houses to which they have been assigned
  2. Ensures the quality of care for all individuals
  3. Provide ongoing 1:1 supervision at least monthly of all staff, including APD
  4. Provides training of all staff
  5. Assists Human Resources in hiring of permanent and relief staff
  6. Provides adequate program coverage
  7. Runs staff meetings at least twice monthly
  8. Provides back up to the residential staff for emergencies and/or arranges for another to be available in their absence
  9. Responsible for staff records to include:
    1. Supervision issues
    2. Evaluation
    3. Time accrued and/or used (re: vacation, sick days)
    4. Trainings
  10. Monitors and accounts for Individuals’ and household finances
  11. Maintains Individuals’ records including daily books, confidential books, financial books, medication books, portable records as well as program communication log in an orderly fashion and in compliance with DDS Licensure and Certification and REACH, Inc.’s specifications
  12. Attends and provides input into Directors’ Meetings or ensures program representation. Disperses information from meetings to all staff in the program
  13. Participate in PBS committees and development, implementation and oversight of PBS techniques
  14. Represents their program and/or individuals when collaborating with the Department of Developmental Disabilities and other state agencies and interfacing providers to include, but not limited to Quality Assurance and Licensing, Transitional Assistance, Housing, and Service Coordinators
  15. Forwarding all required forms to the REACH, Inc. office in a timely fashion to include, but not limited to staff time cards, relief, petty cash and food account records and receipts, change of status forms, complete incident or account
  16. Developing all aspects of individuals’ ISP and ensuring all programs are implemented as specified in this plan. Utilizing HCSIS state system as well creating data sheets and tracking systems
  17. Responsible for state billing of all individuals monthly
  18. Arranging for all necessary medical services for the individuals and following through with all recommendations
  19. Ensures that program staff adheres to program and agency’s policies and procedures and DDS regulations
  20. Screens referrals for Individual vacancies
  21. Establishes and maintains ongoing family contact
  22. Organizes and implements group and individual activities and programs which serve to expand and normalize the lives of Individuals and foster greater degrees of independence
  23. Provides direct care
  24. Provides for a clean, safe home environment for the Individuals
  25. Forwards all required materials to DDS personnel to meet their required timelines
  26. Performs other related work duties as needed or as assigned by the Executive Director, Clinical Director or President

 

 

 

Requirements

Requirements

  • At least 3 years of increasingly more responsible positions with a human service agency
  • Supervisory experience
  • Excellent written, verbal, and interpersonal skills.
  • Must possess a working knowledge of ISP process and Survey & Certification standards.
  • Proficiency utilizing Microsoft Office, video conferencing and the internet
  • Manages multiple priorities and resolves issues in a calm and pro-active manner
  • Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance organizational effectiveness
  • Valid Driver’s License
  • Successful completion of all state and agency mandated trainings including Medication Administration, CPR, First Aid
  • Must be willing to authorize a Criminal History Background Check (CORI), and Fingerprinting check through National Criminal Background Check (NCBC) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies

Must be fluent in English and have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.

Education and Experience

AS or BS in Human Service discipline preferred OR 6 years’ experience in related field.

  • Hours may be altered to meet the needs of the program. Ten calendar days’ notice given.
